Cashew & Pistachio OIT
A targeted oral immunotherapy programme for cashew and pistachio allergy, two closely related tree nuts where allergy often co-occurs and can cause severe reactions.
Cashew allergy is one of the more common tree nut allergies in children and can cause severe reactions even at trace quantities. Because cashew and pistachio belong to the same botanical family (Anacardiaceae) and share closely related proteins, allergy to both is common and is addressed within the same OIT programme.
The programme follows the same principles as other OIT, starting with a tiny measured dose and increasing gradually over months, using cashew flour as the dosing material.
Who is eligible
- Children aged 4–18 with confirmed IgE-mediated cashew or pistachio allergy
- Sensitisation confirmed by skin prick test and/or specific IgE blood test
- Medically stable, able to attend regular clinic reviews
If your child is having a severe allergic reaction, call 999 immediately or go to your nearest A&E. Do not wait. Always carry prescribed emergency medication as directed by your doctor.
Frequently asked questions
Cashew and pistachio share very similar proteins (Ana o 3 and Pis v 1), which means children who are allergic to one are frequently allergic to the other. OIT dosing with cashew typically achieves cross-desensitisation to pistachio.
In most cases programmes are not run simultaneously. Dr Sood will discuss sequencing based on your child's allergy profile and risk.
